Job Overview
We are seeking an experienced Behavioral Coaching and Support Case Manager with a specialty in Substance Use Disorder (SUD) to work with our Custom High Acuity Model (CHAM) team. As a Case Manager, you will provide coaching and support to children and adults with mental health disorders and substance use disorders, empowering them to lead and maintain a healthy lifestyle. You will work closely with our medical partners to assist customers and their families holistically, utilizing a motivational interviewing approach to decrease internal and external barriers that may be preventing individuals from initiating and/or engaging in treatment.
Key Responsibilities
- Perform case management in a telephonic/digital customer-facing environment, utilizing a coaching approach that provides support and resources to customers and family members.
- Develop SMART goals with customers to empower them to lead and maintain a healthy lifestyle.
- Assess readiness to change and implement actions to assist customers in moving through stages of change to achieve their goals.
- Provide services that will assist customers and family members in understanding their options based on their benefits.
- Educate customers and family members about treatment options, including the need for medical interventions (if appropriate).
- Refer customers to Health Professionals with behavioral expertise.
- Ensure that case management program objectives are met by evaluating the effectiveness of alternative care services and maintaining cost-effective, quality care.
- Perform research for community and educational resources to assist customers on their journey.
- Consult with and assist team members in cases where a customer's behavioral health, emotional issues, and/or substance use are impacting their ability to set and achieve goals.
- Actively participate in integrated case rounds, providing insights to promote whole-person health.
- Successfully manage multiple health records and systems to accurately document customer interactions and progress.
- Provide suggestions for improvements and enhancements to outpatient coaching programs based on data and experience.
Essential Qualifications
To be successful in this role, you will need:
- A Master's degree in a mental health discipline and independent licensure to practice in the state of employment (LICSW, LMFT, LPC, LPCC, Licensed Psychologist, or Psych RN).
- Three to five years of post-license mental health and substance use experience preferred, but we will also consider a new, independently licensed therapist.
- Experience with direct customer/client communication (written and verbal), with experience in telephonic counseling/coaching preferred.
- Working knowledge of motivational strategies, materials development, training, and consultation preferred.
- Excellent interpersonal skills and the ability to work in a team environment.
- Proven administrative abilities, with strong computer and software application skills.
- Demonstrated ability to set priorities, including time management and organizational skills.
- A high energy level and excellent written and oral communication skills are essential.
- Passion for health improvement of individuals and family members.
- Strong research and analytical skills.
- Ability to manage own caseload and coordinate all assigned cases.
Preferred Qualifications
While not required, the following qualifications are preferred:
- Experience working with children and adults with mental health disorders and substance use disorders.
- Knowledge of motivational interviewing and coaching techniques.
- Experience working in a telephonic/digital customer-facing environment.
- Familiarity with electronic health records and case management software.

Compensation, Perks, and Benefits
We offer a competitive salary range of $62,400 - $104,000 per year, depending on relevant factors, including experience and geographic location. You will also be eligible to participate in an annual bonus plan and have access to a comprehensive range of benefits, including:
