A company is looking for a Mom and Baby Care Manager - RN who must reside in Nevada.
Key Responsibilities
• Engage members and their support systems through strength-based assessments and trauma-informed care
• Develop and regularly update individualized care plans in collaboration with the inter-disciplinary care team
• Coordinate care and facilitate communication among members, providers, and community resources to achieve health goals
Required Qualifications, Training, and Education
• Nursing degree from an accredited program or Bachelor's degree in a health care field, or equivalent experience is required
• Advanced degree associated with clinical licensure is preferred
• A minimum of three years of experience in nursing, social work, counseling, or healthcare is required
• Three years of Medicaid and/or Medicare managed care experience is preferred
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor is required
