Job Overview
The Population Health/Wellness Case Management Coordinator utilizes critical thinking and professional judgment to support the case management process. This role facilitates and maintains improved healthcare outcomes for members by providing advocacy, collaboration, coordination, support, and education through the use of care management tools and resources. As a telework position, you will be required to travel regionally in-state approximately 5-10% of the time, with a personal vehicle, to meet with members, attend meetings, and participate in training sessions. A valid KY driver's license, proof of vehicle insurance, and reliable transportation are essential for this role.
Key Responsibilities
- Evaluate members through care management assessments and information/data review to recommend approaches for resolving care needs and maintaining optimal health and well-being.
- Identify high-risk factors and service needs that may impact member outcomes and implement early and proactive support interventions.
- Coordinate and implement wellness care plan activities and monitor member care needs.
- Enhance medical appropriateness and quality of care by consulting with case managers, supervisors, Medical Directors, and/or other health programs to overcome barriers to meeting goals and objectives.
- Presents cases at case conferences to obtain multidisciplinary review and achieve optimal outcomes.
- Identify and escalate quality of care issues through established channels and utilize negotiation skills to secure appropriate options and services necessary to meet the member's benefits and/or healthcare needs.
- Utilize influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve an optimum level of health.
- Provide coaching, information, and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices.
- Help members actively and knowledgeably participate with their provider in healthcare decision-making.
- Monitor, evaluate, and document care using case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.
Essential Qualifications
To be successful in this role, you will need:
- Experience and detailed knowledge of the Foster Care and juvenile justice systems, Adoption Assistance, the delivery of Behavior Health Services, Trauma-informed Care, ACEs, Crisis Intervention services, and evidence-based practices applicable to the Kentucky SKY populations.
- At least 2 years of experience in behavioral health, social services, or an appropriate related field equivalent to program focus.
- Computer literacy and demonstrated proficiency in navigating internal/external computer systems and MS Office Suite applications, including Word and Excel.
- Flexibility to work beyond core business hours of Monday-Friday, 8am-5pm, as we are serving the needs of children and families that may require working after school, after work, etc.
- A minimum of a Bachelor's degree or a non-licensed master level clinician in behavioral health or a relevant human services field of study (social work, psychology, marriage and family therapy, counseling).

Compensation, Perks, and Benefits
