This remote work-from-home role requires the ability to work anywhere in the U.S. and will involve working hours of Monday-Friday, 8am-4:30pm Pacific Standard Time. As a Licensed Social Worker, you'll be responsible for researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work to empower members to make positive lifestyle and medical choices and collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health. **Responsibilities** * Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Empower members to make positive lifestyle and medical choices * Collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Utilize a collaborative process to assess, plan, and advocate for members' needs * Develop and implement individualized plans to meet members' needs * Provide education and support to members to promote healthy behaviors and self-management * Collaborate with other departments to ensure seamless care coordination * Maintain accurate and up-to-date records of member interactions and progress **Requirements** * Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in state of residence * 2+ years experience in medical social work or case management * Masters Degree in Social Work * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and critical thinking skills * Ability to maintain confidentiality and handle sensitive information * Strong computer skills and proficiency in electronic medical records **Preferred Qualifications** * Experience working with diverse populations and in a variety of settings * Knowledge of community resources and services * Experience with case management software and electronic medical records * Strong analytical and problem-solving skills * Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities **Benefits** * Competitive salary range: $46,597.20 - $101,228.40 * Comprehensive benefits package, including medical, dental, and vision insurance * 401(k) retirement savings plan and Employee Stock Purchase Plan * Paid Time Off (PTO) and paid holidays * Free development courses and education assistance * CVS store discount and discount programs with participating partners * Fully-paid term life insurance plan and short-term and long-term disability benefits **How to Apply** If you're passionate about delivering enhanced human-centric health care and making a difference in the lives of others, we encourage you to apply for this exciting opportunity.
