Population Health Outreach Specialist

About the position As a Population Health Outreach Specialist, you will play a critical role in empowering communities, connecting individuals to resources, and improving health outcomes by conducting strategic outreach to inform members of 'calls to action' related to recommended tests or screenings or to secure clinical data on behalf of the care team to assess the member's medical, behavioral health and social needs. This position will support quality program management and population health programs and will support personal growth through activities related to program identification, implementation, operations and monitoring. Responsibilities • Organizes information from members' health history and internal data sources to develop an outreach approach to address member needs. • Implement interventions with members and their providers/supports. • Documents member information and contacts made regarding member's care and services provided. • Reviews, monitors, and updates quality program plans, materials and reports. • Actively participates in the creation and distribution of reports related to Quality Programs, Population Health Programs, promoting health, and clinical programs. • Educates members and providers on preventive health guidelines and programs. Requirements • Bachelor's degree in public health, social work, healthcare administration, or a related field. • 2+ years of experience in community health, population health management, or related outreach roles. • Strong understanding of social determinants of health and health equity principles. • Exceptional communication and interpersonal skills to engage diverse audiences. • Proven track record of program planning, implementation, and evaluation. • Able to learn documentation systems such as Epic. • Knowledge of Medicare and Medicaid benefits and regulations. • Experience with Motivational Interviewing and Trauma informed care. • Knowledge of local, state, and federal community resources available to members.
