Description We are seeking a compassionate and skilled Registered Nurse (RN) to join our remote team for a 6-month contract as a Care Coordinator/Case Manager. In this role, you will be instrumental in assessing the health and care needs of individuals, particularly those in need of long-term care or support due to chronic illnesses or aging. Your expertise will directly impact the quality of care provided to clients, as you conduct telephonic assessments, collaborate with field assessors and facility staff, and update care plans to ensure that each individual’s needs are met. Skills • Active RN license in good standing (Compact license preferred). • 2-5 years of experience in case management, home care, hospice, or rehabilitation settings, particularly with aging populations. • Proficiency in Microsoft Office applications (Outlook, Word) and familiarity with workflow management tools and CRM systems. • Ability to work independently in a remote setting and manage multiple tasks efficiently. • Comfortable with a 6-month contract position, with the potential for an extension or permanent offer. Responsibilities • Review and analyze clinical assessments from field assessors to assess chronic illness status and develop comprehensive plans of care. • Collaborate with field assessors and facility staff via phone to gather additional health information when necessary. • Conduct telephonic assessments with claimants, their family members, or facility staff to update health records, create new care plans, and document changes. • Work closely with new clients to develop processes that align with the needs of their insured individuals. • Ensure timely and accurate documentation of care plans and changes to health status in electronic health records. Qualifications • Active RN license in good standing • 2-5 years of relevant experience in case management, home care, hospice, or rehabilitation settings. Education Requirements • Associate’s Degree in Nursing (BSN preferred).
