Job Summary: We're looking for a full-time, bilingual (English and Spanish), and remote LVN/LPN to provide high-quality patient care, coaching, and appointment scheduling via phone. As a Care Coordinator, you will: Remotely deliver patient health coaching and care to improve patient outcomes Coach and educate patients on managing chronic conditions, preventive care, and physician directives Update and manage patient care plans based on assessments and physician directives Maintain scheduling flexibility between 8:00am - 7:00pm (PST) to accommodate patients in different time zones Requirements: Valid LVN/LPN license in the state of New York (mandatory) 2+ years of experience as a Licensed Vocational Nurse (LVN/LPN) Bilingual proficiency in English and Spanish Experience working with Electronic Health Records (EHRs) and Electronic Medical Records (EMRs) Strong knowledge and background in chronic conditions Excellent patient rapport-building skills Compassionate and caring demeanor Strong documentation and verbal communication skills Intermediate to advanced computer skills with the ability to navigate multiple systems Reliable internet connection Apply Now to join our team and start making a difference in the lives of our patients!
