Make an impact as an LPN Community Health Specialist! You will be the driving force behind our Population Health mission—using data-driven strategies and creative wellness initiatives to transform lives. From one-on-one health coaching and clinical education to high-energy community outreach, you'll empower PrimeWest Health members to take charge of their health and crush their personal wellness goals!
Key Responsibilities
- Clinical Intervention: Conduct health coaching and follow-up calls; accompany members to appointments and facilitate communication between providers and the interdisciplinary care team.
- Program Administration: Manage Focused Wellness programs and implement data-driven strategies under the direction of the Population Health Manager and Medical Director.
- Community Engagement: Represent PrimeWest Health at community events and health fairs to build partnerships and identify member needs.
- Documentation & Reporting: Maintain accurate electronic medical records (EMR), track progress in care management software, and assist with annual satisfaction surveys.
- Compliance: Ensure all duties align with DHS and CMS regulatory requirements and PrimeWest Health initiative measures.

Educational Requirements and Required Qualifications
1. At minimum, Licensed Practical Nurse (LPN) diploma
2. Demonstrated knowledge of, and proficiency in, Word, Excel, and Outlook required. Experience in relational databases and business information systems is a plus.
3. Flexibility and ability to handle multiple tasks. Demonstrated organizational skills and attention to detail. Ability to work effectively with a variety of people both within and outside of the organization. Good verbal and written communication skills.
4. Ability to sit at a desk or computer station most of the time. Ability to occasionally lift and/or carry such articles as file folders and small boxes up to 30 pounds.
5. Ability to travel approximately 20 to 50 percent of the time, including limited overnight travel.
6. Experience in public speaking and working with groups of people preferred.
PrimeWest Health’s corporate office is in Alexandria, Minnesota. However, this position is primarily remote, serving one of the following regions. The candidate must reside in one of the listed counties. This position works from home when not in the field.
- Southwest Minnesota counties: Cottonwood, Jackson, Lincoln, Lyon, Murray, Nobles, Pipestone, and Redwood
- Northern Minnesota counties: Beltrami, Clearwater, and Hubbard
- Central Minnesota counties: Meeker and McLeod
