Description Hospice RN On-Call – Rotating Extended Shift Schedule (Full Time) VNA Hospice of Northwest Indiana (NWI) Counties Served: Porter, LaPorte, Jasper, Lake, Newton & Starke Position Overview VNA Hospice NWI is seeking a dedicated and compassionate Registered Nurse to join our hospice team under a rotating extended-shift schedule. This position follows a 6-week repeating rotation that includes combinations of 8-hour, 16-hour, and 24-hour shifts on call. The schedule is designed to provide predictable long-range planning while supporting excellent continuity of care for hospice patients. This role is ideal for an RN who thrives in a flexible, structured rotation and enjoys working autonomously in the field. Requirements  General Schedule Pattern Across the 6-week rotation, the RN will work: - A blend of day, evening, and overnight extended shifts - Approximately 3–4 shifts per week, depending on shift length - No traditional Monday–Friday requirement - No mandatory weekly on-call, as extended shifts provide built-in coverage - A predictable repeating cycle that allows long-term planning - - Key Responsibilities - Provide compassionate, high-quality hospice nursing care in patient homes - Complete assessments, symptom management, and care planning - Collaborate with interdisciplinary team members - Support families through education and emotional guidance - Document care accurately and promptly - Maintain autonomy while managing a field-based caseload - - Qualifications - Active Indiana RN license - Hospice or home health experience preferred - Strong clinical judgment and comfort working independently - Valid driver’s license and reliable transportation - Ability to work extended shifts within a rotating schedule - - Benefits - Mileage reimbursement - Medical, dental, vision, life insurance, and Disability insurance offered - Paid time off & holidays - 403(b) retirement plan - Supportive, mission-driven team environment Why Join VNA Hospice NWI?
