Job Overview: We're seeking a dedicated and empathetic Field-Based HSS Clinical Coordinator Registered Nurse to join our team at UnitedHealthcare. As a key member of our Long-Term Services and Supports (LTSS) team, you will play a vital role in coordinating care for individuals with complex medical needs, ensuring they receive personalized, effective, and efficient support. Key Responsibilities: Develop and implement individualized care strategies, focusing on person-centered, goal-oriented, and culturally relevant support Conduct home visits and in-person assessments to identify and address social determinants of health, ensuring seamless care coordination and integration into the community Collaborate with healthcare teams, community service coordinators, and stakeholders to provide high-quality, coordinated care and services Advocate for individuals and families, ensuring their needs and choices are fully represented and supported Utilize MS Word, Excel, and Outlook to manage care plans, track progress, and maintain accurate records What You'll Need: Bachelor's Degree (or higher) in Social Work, Rehabilitation, Nursing, Psychology, Special Education, Gerontology, or a related human services field 2+ years of experience working in community health settings, with a focus on healthcare roles 1+ year of experience working with individuals with long-term care needs, including those receiving services on one of the homes and community-based waivers in KS 1+ year of experience working with MS Word, Excel, and Outlook Ability to travel within a 30-60 mile radius, 2-3 days per week Preferred Qualifications: Licensed Social Worker or clinical degree Registered Nurse Background in managing populations with complex medical or behavioral needs Experience with electronic charting and arranging community resources What We Offer: Competitive salary range: $28.61 to $56.06 per hour (based on full-time employment) Comprehensive benefits package, including incentive and recognition programs Equity stock purchase and 401k contribution (subject to eligibility requirements) Opportunities for professional growth and development in a dynamic, supportive environment Join Our Team: At UnitedHealthcare, we're committed to creating a healthier, more equitable world for all.
