Community Care is hiring a Case Manager/Care Manager to join our Walworth team. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. It is based in Walworth County with telework currently being the norm.
Case Manager/Care Manager Job Responsibilities:
- Manages an assigned caseload of program members in collaboration with an RN Care Manager
- Completes comprehensive member assessments, member centered care plans, & functional screens
- Provides ongoing care coordination and quality assurance monitoring of services
- Maintains required documentation in adherence to established standards.
Case Manager/Care Manager Job Requirements:
Education: Bachelor’s Degree in a Human Services Area required.
Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity required.
Certification: Eligibility for long-term care functional screener certification is required
Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Care Manager Job Schedule:
- Monday to Friday
- Generally during business hours of operation (8AM-4:30PM)
